You're eating well. You're moving more. You're doing everything the advice says — and the scales refuse to move. Or worse, they keep creeping up. If this sounds familiar, here's something important: weight is not just a willpower equation. It's a hormonal one.
For women — especially between 35 and 65 — hormones sit at the control panel of metabolism. When they're out of balance, the body actively resists weight loss, no matter how disciplined you are. Understanding this changes everything about how you approach it.
The thyroid: your metabolic thermostat
Your thyroid — a small butterfly-shaped gland at the base of your neck — produces hormones that set the metabolic rate of essentially every cell in your body. Think of it as the thermostat that decides how fast you burn energy.
When thyroid function drops (hypothyroidism), metabolism slows. Calories that once fuelled your day are now stored. Alongside weight gain, an under-active thyroid commonly causes:
- Persistent tiredness and sluggishness
- Feeling cold when others aren't
- Brain fog and low mood
- Hair thinning and dry skin
- Constipation and fluid retention
Women are several times more likely than men to develop thyroid dysfunction, and the risk rises with age — exactly when many women are also navigating perimenopause, which muddies the symptom picture even further.

Insulin: the storage hormone
Insulin's job is to move sugar from your blood into your cells for energy. Cortisol: the stress saboteur
Cortisol — your primary stress hormone — evolved to help you survive short-term threats. Through perimenopause and menopause, estrogen and progesterone decline — but not smoothly, and not together. This hormonal turbulence changes how and where your body stores fat, shifting it from hips and thighs to the abdomen. Lower estrogen also reduces insulin sensitivity and can disrupt sleep — and poor sleep independently drives weight gain by increasing hunger hormones.
Many women describe gaining 5–10 kilograms through this transition without changing anything about how they eat or move. That's not a discipline failure. It's biology — and it can be addressed.
Why standard diets fail hormonal weight gain
Conventional advice — "eat less, move more" — assumes a normally functioning metabolism. But when thyroid output is low, insulin is high, cortisol is elevated and estrogen is shifting, severe calorie restriction can actually make things worse: slowing thyroid function further, raising stress hormones and triggering rebound weight gain.
This is why the right sequence matters:
- Test first. Identify which hormonal systems are actually involved — thyroid, insulin, cortisol, sex hormones, or several at once.
- Treat the root cause. Optimise thyroid function, restore insulin sensitivity, regulate stress and support the menopause transition where needed.
